The lack of a fall in the maternal death rate in the latest report by the Confidential Enquiry into Maternal and Child Health (CEMACH) has been partly attributed to the increasing percentage of births from immigrant women. Every nurse in the worls can tell you horror stories about being in the nursing school. My experience is not any different from what other nurses across the globe. I still remember my first day in a clinical rotation in a VA hospital.
The horror started from the night before the first rotation. I had to research on about 50 different medications and prepare the so called "drug cards" before the next day and be in my clinical site by 0630am. The list for these drug cxards was given to us the night before and the instructor expected them complete before the shift start. I had to stay on the whole night doing this and by the time I finished it was time to take a shower and prepare to go so as not to be there 5 minutes late that would have led to a written warning.
My "drug cards" were complete on time and I got there on time after 33 miles drive. I needed caffein so badly that I passed through gas station and bought caffein pills called "stay awake". These pills did their work and I stayed awake throughout but I had a terrible heart burn and palpitations that were almost as bad as a panic attack.
I was assigned to go to the operating room, populary known as OR in any hospital. I had never seen any surgery before other than on TV. It was a moment that I still remember. The procedure was an open heart surgery callled "coronary artery bipass graft" or as it is called in medical field, "cabbage". I was optimistic and excited to watch such a complicated procedure being done by expert cardilogists.
...But I passed out and became a patient for some good 15 minutes
The surgeon cardilogist was very friendly Indian guy who has a talent to teach and make the OR a fun place. He took a blade and made the first incision. I remember seeing blood coming out and saw ceiling lings and darkness. I passed out!
When I regained conciousness, I was leaning lying on a OR nurse chair ans everyone was making fun of "the first experience passout". Turned out that it is a common thing for those who have never seen a surgery before or something close to that. I got up, this time feeling a little dizzy and continued watching the surgery. This time the surgery had progressed and was time to rip the sternum apart with a power saw like tool.
... And that was just the introduction, day #1
The surgery took about 4-6 hours and I couldn't wait to get out of there. It seemed to me like it took the whole day and was expecting to see "darkness" outside, only to find day light at noon. My first day in as a student nurse still remains a day to be remembered, but not the only day I can talk about. Many more fun days and other horrible ones were just ahead of me. An especially special friend of mine spoke with me about the uselessness of primary care physicians. And almost had me, but something came to mind when I read an article in the July 2, 2008 edition of the JAMA (Journal of the American Medical Association) by Dr. Michael Stillman. My friend presented me with the idea that general care physicians are not always very knowledgable about ailments or certain health problems that specialists would know. I had a personal experience that comes under this category. However, when the patient of a primary care physician is ill or injured, the primary care physician will know the patient well. One of the most important aspects of health care that is commonly overlooked is knowing the patient. Often such luxury is not possible with the busy schedule of doctors, or the lack of need to see certain doctors. Nonetheless, it can influence care dramatically. Dr. Stillman gave an example in his article where the medications of a patient had been changed to a more expensive regimen without his knowledge. The prescribing doctor would not have know that the patient could never afford those medications, but Dr. Stillman knew and prescribed the original, less-costly, and equally effective regimen. A number of other issues come up as well when identifying the importance of primary care physicians. When making an important medical decision, trust between a patient and the doctor becomes essential. Patients, even if are under the care of a specialist may want to consult a phyiscian with whom they are familiar before making an important decision. Primary Care doctors may not necessarily save lifes, but they can certian help people maintain the highest quality of life possible. So with the greatest level of respect possible, I disagree with my friend [I will convince her I am right, and if she happens to read this before I try the art of persuasion...] (citation: JAMA July 2, 2008 p.21-22).
I have good news and even better news but first we have to take a detour through bad news valley.
Last Thursday, the pediatrician phoned to let us know that the thyroid portion of the PKU newborn screen had come back abnormal. She wanted us in her office that afternoon to speak with us and get additional tests completed to rule out . The rush was because the longer Congenital hypythyroidism goes untreated, the higher the risk of mental developmental delays and other delays.
We went in and she explained to us the tests and we also checked his weight. In a little under two weeks, he went from 12lbs 7oz all the way up to 14lbs 1oz!! The lab was next door and he had his blood drawn. Then we waited.
Three of the five tests came back with normal results by the next morning. I cried.
This past Tuesday I took him for his original weight check appointment (he gained another 8oz by the way!) and the ped had the other two results. They too were normal. The endocrinologist does not need to see him and we go back in three weeks to follow up.
We are no longer breastfeeding and he is completely on formula. After everything i went through with Charlotte and my PPD, I know my limits and pumping or troubleshooting why nursing wasn't working is beyond my scope this time around. He did nurse yesterday morning and I will continue to let him nurse as I need him to or as he wants to until my supply has ceased. Even when I'm giving him a bottle we are cuddly and have eye contact. It doesn't matter what goes into his stomach or how it gets there. All that matters is that we're thriving, happy, and healthy.
But there are something things that I find to help me stay sane while searching:
Keep an open mind. Anything is possible. It might not be the dream job, but opportunities are everywhere.
Don't cut yourself short. Even the jobs that seem impossible, dream big! Apply...it can't hurt. At least now the agency will have your resume on file!
Look where no one has dared looked before. If you went to school to be a nurse. Chances are you are going to look to work in a clinic or hospital. But what about cruises as a on-site caregiver? Or at a school or summer camp? Typically when we get out of school, we look to the places where we were trained to look---do what your teachers told you: think outside the box (chances are there are spaces available).
Stock up on water, munchies, paper and ink. It's going to be a long search--be prepared and bring your essentials for the trip.
